Hospice Care in Your Home

Hospice care allows the patient to remain in their home environment surrounded by family and friends through the end of life.

The care needs of patients who are nearing the end of life normally become more intense. This may mean caregivers are no longer able to meet their loved one’s care requirements without additional support. Hospice assistance can mean that caregivers do not have to choose between hospitalization or nursing home care during their loved one’s last months.

With the supportive care that our hospice team provides, hospitalizations at the end of life are usually completely avoided. Often, with hospice care, the patient is able to remain in their current residence until death – whether it is a personal home, group home, assisted living facility or nursing home. This is their home – the place they know and are comfortable and where they have caregivers they know and trust. Hospice can help patients spend their final days in a familiar environment surrounded by those they love.
